troublesome to you (if you have pains in several different
parts of the body).
· Symptoms associated
with your most troublesome pain.
· Limitation of daily
activities caused by your most troublesome pain.
· Email address (to send
your Personal Pain Report).
· Age, Gender, Name,
Mailing Address. (You don’t have to provide these, but they
will help us to interpret the study results. We take all
legal means to protect your confidentiality and prevent
unsolicited contacts.)
