• TMT provides, as a service to the public, medical articles on the most common chronic diseases, including the main causes of chronic pain. 
• If you answer our Chronic Pain Survey, TMT will provide you with a set of articles on chronic pain selected by computer on the basis of your symptoms, as well as a Personal Pain Report based on the symptoms you describe (click the 'Pain Survey' button above).
• If you answer our Chronic Disease Questionnaire, TMT will provide you with computer-selected articles and a Personal Medical Report.

You don't have to submit a Chronic Pain Survey or a Chronic Disease Questionnaire in order to read the medical articles on chronic pain and chronic diseases. These articles are provided as a public service by TMT. 

If you do send us your Chronic Pain Survey or Chronic Disease Questionnaire, our computer will analyze your responses and provide you with those articles that seem appropriate for you.  You do not have to provide your name or other personally identifying information but you do have to provide an email address so that we can communicate with you.
