Standard web navigation should be used for the medical questionnaires. A "Continue"button is provided at the foot of each page for a questionnaire - click this to go to the next page. When you click the "Continue" button on the last page, you will be shown several report options (e.g., a Brief report, a Full report, a report for your Doctor, or a report for your Emergency Medical Card to store in your wallet or purse). General Information for Medical Questionnaires TMT's not-for-profit division empowers patients by: 1. A set of free questionnaires for individual symptoms/medical conditions that ask the questions an expert doctor might ask. 2. A free Comprehensive Personal Medical Report questionnaire that automatically calculates cardiovascular risk based on your values of cholesterol (and other blood lipids) and your waist/hip ratio. You can copy, print or email your personal reports (usually a full report for your records, and a report suitable for your doctor), and can save a copy on your own computer. These reports are intended to make the discussions with your doctor more productive, and you are advised not to use them for self-diagnosis. The report for your doctor is intended to be used together with your doctor's additional questions, physical examination and diagnostic tests. You (or your doctor with your permission) can access your report from the Internet anywhere in the world at any time of the day or night. When national standards emerge from the current mix of acronyms (XML, HL7, CCR, etc.), we will modify our reports to be totally or substantially in compliance with these new standards - so that the reports for your previously completed questionnaires will then be automatically updated when you access them. We will not provide your information to third parties without your prior permission. We advise use of a password for additional security of your medical data and you can also answer a questionnaire anonymously. We use Secure Sockets Layer technology for Internet data transfer. Some, but not all, TMT's questionnaires are also protected under a Confidentiality Certificate (CC) from the US Government's National Institutes of Health. The TMT Comprehensive Personal Medical Report prompts you (or your healthcare provider) for your key medical information - Symptoms by body system (e.g., cardiovascular), Symptoms by body location, Current medications, Immunizations, Allergies, Past Medical Conditions, Previous Surgeries, Previous Hospitalizations, Diagnostics Tests & Clinical Measurements, Social History and Family History. Provide as much or as little information as you want - you can even answer anonymously and fill in your personally identifying information when you get your reports. Various risk factors are automatically calculated. Access or update the information any time of the day or night from anywhere in the world. TMT condenses your information into three different reports. One report is for an Emergency Medical Card for your wallet or purse. One is for your doctor. One is a full version for your records. The Pain Types questionnaire asks the questions a pain specialist might ask to determine what type of pain you have: Nociceptive, Neuropathic, Visceral, or Psychogenic -- or Mixed Pain (more than one pain type). Identification of the pain type (or pain types) in a given person is important for both diagnosis and treatment. The Lower Back Pain/Sciatica questionnaire asks the questions an expert might ask about lower back problems (the second most common reason Americans see their doctor). The reports summarize your responses and any evidence for a neuropathic component to your pain, and provides medical information on back pain & sciatica. The Knee Pain questionnaire asks the questions an expert might ask about knee problems. For the reports, TMT's software evaluates the diagnostic significance of your responses and summarizes the evidence for each common knee condition.
The Shoulder Pain questionnaire asks the questions an expert might ask about shoulder problems. For the reports, TMT's software evaluates the diagnostic significance of your responses and summarizes the evidence for each common shoulder condition.
Other pain questionnaires are for hip pain, toothache, jaw pain/temporomandibular joint disorder, chest pain, peripheral neuropathy, and wrist pain/carpal tunnel syndrome. Other questionnaires address symptoms (e.g., feeling ill, edema, fever, sleeping problems), diseases (e.g., depression, BPH, visual field defects) or other medical issues such as flu shots and clinical trial enrollment. After the Hurricane Katrina disaster, TMT offered free computer and medical support for hurricane evacuees. One of TMT's services is an electronic version of the paper KIWY medical form, provided by the Centers for Disease Control (CDC), to be used for evacuees from previous or future disasters.
The Pain Survey is an IRB-approved TMT research study of how pain location can help in the diagnosis of chronic pain. As an incentive for you to participate, we will give you a Personal Pain Report and a personalized selection of articles on chronic pain. We provide a list of FAQs (Frequently Asked Questions) for the Pain Survey. Y ou can update your confidential medical records at any time.Most of our questionnaires allow you to draw your symptoms on a diagram of the human body. This helps your doctor diagnose your condition, and helps our ongoing computer imaging research into the diagnostic value of disease location.Save, print or email your personal medical reports: When you get your medical report from our data-secure server, you can save your report to your own computer, print a copy, or email a copy to your doctor or anyone else. YOU have control. Access on the internet any time from anywhere in the world. We give you a unique Internet address so that you (or your doctor with your permission) can access your reports from the Internet anywhere in the world at any time of the day or night. You can provide your medical information anonymously Your choice - anonymity or limited disclosure of personal information: You have the option of providing your medical information anonymously. This gives you complete protection against unauthorized disclosure of your medical information -- since only YOU know to whom the information applies. You also have the option of providing limited personal information so that this can be included in your Personal Medical Report. Since there is a very small chance that someone could improperly get the Internet link to your Personal Medical Report, we suggest you provide a password for additional protection. Privacy We make extensive efforts to protect your personally identifying information but cannot absolutely guarantee such protection. Accordingly, you should weigh carefully the advantages and disadvantages of providing personal information. We do not provide this information to third parties without your prior permission. Questionnaire software free to non-profit entities TMT will provide the software for most questionnaires at no charge to non-profit entities such as hospitals or national, state or local governments.
